Welcome to Nextera Health! I am Dr. Opara, a native of the D.C., Maryland, and Virginia area, and I am proud to serve the Nextera community! While I was raised here, I honed my hands-on style of practicing family medicine while training in rural Pennsylvania, which taught me the value of a strong patient-doctor relationship.

As a first-generation child of Nigerian immigrants, I was raised with a deep-seated appreciation for hard work, and I bring that commitment to every patient interaction. I chose a career in family medicine not just as a profession, but as a calling—a way to live out my Christian faith by showing the love of Christ through healing and caring for the sick. My passion is to help my patients regain autonomy over their health, especially those struggling with obesity and related chronic illnesses. Through the Direct Primary Care (DPC) model, I have the time to truly listen to your story and use my expertise in lifestyle and obesity medicine to partner with you on a personalized wellness journey. I practice evidence based care and my goal is to build a healthcare safety net that is accessible, affordable, and dedicated to your long-term well-being.

When I'm not in the office, you can find me having dance parties with my six-year-old nephew, laughing until I can't breathe with a close group of friends, or "rucking" with my local hike club. These moments with family and friends fuel my spirit and reinforce the importance of living a vibrant, active life.

Sarah Oelke, FNP-C

Sarah has been a board-certified Family Nurse Practitioner since 2012. Her nursing career began in critical care, with experience in the ICU, step-down units and labor & delivery. As a nurse practitioner, she has worked in gynecology, oncology, pediatric surgery, and pediatric urgent care. For the past nine years, she has focused on pediatric primary care, providing comprehensive healthcare to children and their families. At Nextera Healthcare, Sarah looks forward to treating patients of all ages and is committed to delivering compassionate, evidence-based care across the lifespan.

Sarah is passionate about building strong patient-provider relationships and strives to create a welcoming environment where individuals feel heard and valued. She believes in a collaborative approach to healthcare, ensuring patients leave each visit feeling informed and empowered rather than lectured.

She completed her undergraduate studies in biomedical zoology at the University of Oklahoma before earning her Bachelor of Science in Nursing at Oklahoma City University, where she was a member of the Sigma Theta Tau International Honor Society of Nursing. She went on to earn her Master of Science in Nursing as a Family Nurse Practitioner from Saint Louis University.

Outside of work, Sarah enjoys spending time with her husband and their two sons, ages 10 and 12. Their family also includes a beloved giant doodle dog named Gander. She trains in taekwondo, is an avid OK Sooners football fan, enjoys outdoor activities, and loves to travel.

Matthew Haden, M.D.

Matt Haden, MD, was born and raised in Nebraska. He completed his Bachelor of Science at Nebraska Wesleyan University. He then attended the University of Connecticut School of Medicine for a joint MD and Masters of Public Health program. Dr. Haden completed 18 months of residency training at Georgetown/Providence Hospital in Washington, DC before transferring to the Mayo Clinic Arizona, where he completed his specialty training in Family Medicine. He is board certified in Family Medicine and has worked for several years in urgent care and primary care.

While working as a doctor, he also went to business school and obtained his MBA to help in his entrepreneurial endeavors. He was the founder and CEO of MobileMDs, an on-demand service for doctor’s house calls serving the urgent care needs of travelers across the US. After several years of growing that business, he decided to return his focus to primary care and restoring the doctor-patient relationship through Direct Primary Care (DPC).

Dr. Haden has traveled extensively and even lived abroad for a time in Egypt and Costa Rica. Over the past 20+ years, he has enjoyed the opportunity to visit Rome, Casablanca, Malta, Gibraltar, Montreal, Ottawa, several parts of India, Nepal, Mexico, Trinidad, and the Dominican Republic. These experiences have helped him to better understand how one’s culture can influence health and treatment preferences of patients.

Dr. Haden enjoys working out, visiting family, drinking coffee, playing the drums, driving his electric car, and spending quality time with his wife and daughter. He was selected as a 2015-2016 Fellow of the Leonine Forum at the Catholic Information Center in Washington, DC and volunteers with the Order of Malta Federal Association Auxiliary.

John-Paul Jansen, MD

Dr. Jansen grew up in rural Quebec and later moved the US for college. He attended Christendom College and later earned his Doctoral degree in medicine at West Virginia University. School of Medicine. He completed residency at Kansas University for Internal Medicine. He initially practiced as a hospitalist in the Denver metropolitan area at several large hospital settings.

He and his family moved to Front Royal in 2017, first working at Winchester Medical Center. Passionate about good, honest and personalized health care, he joined his father in starting a DPC practice in 2022. His lives with wife, Anne, and five children in Front Royal, VA.

Joseph Norton, FNP, NP-C

Joseph Norton is a board-certified family nurse practitioner. He lives in Front Royal, Virginia with his wife and five children. He completed his nursing studies at George Mason University and holds a bachelor's degree in philosophy from Christendom College. He earned his master's degree in nursing practice from Drexel University in 2023. Before joining Fiat he was a nurse practitioner in urgent care, where he took care of adult and pediatric patients. His nursing experience includes intensive care and emergency room.

Informed by this extensive acute care background, he wants to help patients prevent serious illness. He believes this is only attainable through a personal approach to medicine that emphasizes bodily stewardship. Joseph considers himself blessed to practice medicine in the beautiful Shenandoah Valley.

John-Paul Jansen, MD

Patrick Vander Woude, FNP, NP-C

Patrick has practiced in family practice and most recently pediatrics. He completed undergraduate training in Nursing at Bellarmine University in Louisville, Kentucky, and completed his Masters in Nursing at George Washington University in D.C. Before practicing medicine, he served as a pilot in the United States Marine Corps.

Patrick graduated from Christendom College in 2005, where he met his wife. He lives on a small farm in Nokesville, Virginia, with his wife Jill, and seven children. Patrick is dedicated to medical freedom, informed consent, and working to practice medicine in a God-centered, holistic manner.

John Grimsley, M.D. (Closed to New Members)

Dr John is a father of 5 children, husband and a family physician. He grew up in Northern Virginia and has spent most of his life here. In his youth, he pursued athletics (wrestling, football, boxing and more…) and developed an interest in human physiology and philosophy. Following his undergraduate work at George Mason University (GMU) he completed a Master’s in Biology at GMU. Thereafter, he worked in the biotech industry for Human Genome Sciences in their Oncology division. This experience eventually lead him to understand his true passion - helping others achieve a long and healthy life.

After completing residency, he started Vantage Primary Care. A private practice that cared for patients admitted to Fairfax Hospital. During this time, he learned a lot including how health systems really operate and how healthcare consolidation harms the patient.

Dr John and Diana started Summit Direct Care as they believe a better approach to health was truly needed. One of the issues facing our society is over medication and lack of a root cause approach to health. The problem is few doctors have the time, incentive or knowledge to help patients reverse disease or build a strong health foundation. Summit Direct Care was founded so you can have a doctor with the time, knowledge and desire to help you craft a personalized plan to reverse and prevent disease.

Dr John works with his patients in a collaborative manner, to get to the root cause of their problem(s), then creates a detailed and personalized plan. He focuses his efforts on optimizing your health through lifestyle and reversing chronic disease whenever possible. He practices broad spectrum family medicine (care for all ages) and has a passion for musculoskeletal medicine, wellness/preventative health, lifestyle medicine and nutrition.

Mary Alling, M.D.

Dr. Mary is committed to a holistic approach to medicine. As an undergraduate at the University of Scranton, she took to heart the motto of “Cura Personalis,” or “care of the whole person,” and uses this as her approach to patients.

Dr. Mary grew up in Fredericksburg, Virginia. Upon graduating with a bachelor’s degree in biology and international studies, she attended medical school at Loyola University Chicago Stritch School of Medicine while in the Navy Reserves. She then transitioned to active duty in the Navy and completed her Family Medicine residency at Naval Hospital Jacksonville in Florida. During this time, she found her passion in Lifestyle Medicine, an approach to medicine used to prevent and treat medical conditions through healthy life choices. After her residency, Dr. Mary was stationed at the Mountain Warfare Training Center in Bridgeport, California, where she completed her service in the Navy. After experiencing various medical facilities, she wanted to find a better environment to treat, educate, and empower patients adequately. Thus she is excited to work at Summit Direct Care, where relationships can be developed with patients and help them work toward their healthiest selves.

Outside of the office, you can typically find her training for endurance events (such as a triathlon or a multi-day bike trip) and running agility with her dog. She is excited to be back in Virginia and is happy to be exploring the Appalachians again.

Ryan Lawson, FNP

Ryan is driven by a passion for providing comprehensive care. His approach to healthcare is holistic and collaborative. He believes in empowering patients to take an active role in their health, fostering a patient-centered environment where shared goals include improving wellness, preventing and managing disease, and enhancing quality of life.

Ryan earned his Bachelor of Science in Nursing from Marymount University in 2016 and began his healthcare career as a trauma intensive care nurse at Fairfax Hospital. Building on this experience, Ryan pursued advanced education and obtained his Master of Science in Nursing from James Madison University in 2020, specializing as a Family Nurse Practitioner.

A husband and father of three, Ryan shares a deep connection with his roots in Northern Virginia, where he grew up enjoying sports and outdoor activities. His athletic pursuits included football, baseball, track and field, and wrestling. Today, he continues to embrace the outdoors through hunting, fishing, camping, hiking, and canoeing, but his greatest joy comes from spending time with his wife and children. Faith and family remain the central pillars of his life.

Before joining the Summit Direct Care team, Ryan worked for nearly five years as a family practitioner. While he valued his time in his previous practice, he became increasingly aware of the limitations posed by brief appointments and insurance hurdles, which often hindered optimal patient outcomes. At Summit, Ryan is excited to focus on delivering truly personalized care, free from these constraints, and building strong partnerships with his patients.

Sarah Gilpatrick, DNP, FNP-C

Sarah received her Doctorate of Nursing Practice in Family Medicine at the University of Maryland. Sarah has a passion for holistic women’s health, restorative reproductive medicine, and all things hormones. She teaches the Marquette Method of NFP to women and couples, particularly those who are post-partum and/or breastfeeding. She is a member of the Marquette Method Professional Association and the International Institute for Restorative Reproductive Medicine.

Sarah lives in Centreville, VA with her husband and two daughters.

Nathaniel Arnatt, NP-C

Nathaniel Arnatt is a licensed and board certified family nurse practitioner and the owner of Simply Sweet Health. Growing up in Spotsylvania, he attended Spotsylvania Middle School and Fredericksburg Academy before earning his bachelor's degree from Hampden-Sydney College in 2005.

In 2008 he answered a calling to become a registered nurse and graduated with honors from Germanna Community College in 2010.

Nathaniel Graduated among the top of his class from The George Washington University in 2017 and began working for The University of Virginia.

As a family nurse practitioner, Nathaniel has gained experience in family medicine, internal medicine, nursing home, emergency medicine, and urgent care.

Diksha Gupta, D.O.

Dr. Diksha Gupta, a Fairfax, Virginia native and a passionate healthcare provider, is delighted to offer her services to the NOVA region. Elizabeth Weaver, M.D.

Elizabeth Weaver, MD, is a fellow of the American Academy of Family Practice and is a diplomate to the American Board of Integrative Holistic Medicine. She loves the breadth of issues to tackle in family medicine and its focus on prevention. She has a special love of working with adolescents.

As a board-certified family physician practicing for 17 years she has witnessed many changes in the way health care is delivered.

She has always held the belief that good primary care physicians recognize that each patient is an individual. Good patient care involves searching for the underlying, root cause of disease, not simply the treatment of a myriad of symptoms.

Spencer Dean, D.O.

Dr. Dean is a board certified, Virginia licensed, Family Medicine physician who has been practicing for just under 10 years. He received his undergraduate degree at Brigham Young University in exercise science with a special interest in sports medicine. He was selected as a recipient of the health professions scholarship in the United States Navy and commissioned as an officer in 2015.

Dr Dean attended Midwestern University Arizona College of Osteopathic Medicine and graduated in the top third of his class. He completed his family medicine residency at the Naval Hospital at the Naval Air Station in Jacksonville, Florida. During those years he also worked in various civilian hospitals and clinics throughout the region.

As a resident, Dr Dean received the Resident Scholar Award given to the resident with the most published research. Most of his research has focused on the uses of Ultrasound as a diagnostic tool in musculoskeletal injuries and as an adjunct tool to achieve better therapeutic results with joint therapies and injections. He received additional training in the use of ultrasound for these therapies and employs them in patient care today. He looks forward to adding ultrasound to one of Real Health’s offered services in the near future.

Dr. Dean loves to spend his free time with his wife and their three children. He enjoys good music, the outdoors, hiking, cycling, mountain biking, races, history, and lacrosse.

Reynaldo Lee-llacer, M.D.

Reynaldo Lee-Llacer II, M.D. (Dr. Rey) graduated from the University of Maryland Baltimore County with a Bachelor of Arts in Psychology. He earned his doctorate in medicine at De La Salle University in the Philippines and completed his Internal Medicine training at University of Washington Spokane Internal Medicine.

Dr. Rey has over 20 years of experience. He has worked in outpatient primary care, skilled nursing facilities, hospital based medicine, wound care, and as a specialist in hair restoration surgery.

Outside of work, Dr. Rey loves spending time with his family. He enjoys ballroom dancing and watching kdramas with his wife, listening to his kids’ viola performances, and exploring different cuisines.

David Bigley, D.O.

Dr. David Bigley, DO, embarked on his journey in healthcare with a strong foundation in neuroscience and biochemistry, earning his undergraduate degree from the University of Scranton. Inspired by a holistic approach to medicine, he pursued his medical education at the Philadelphia College of Osteopathic Medicine (PCOM) in Philadelphia, where he embraced the principle of cura personalis – caring for the whole person.

During his residency in Family Medicine at Christiana Care in Delaware, Dr. Bigley honed his skills while understanding the limitations of the current insurance-based primary care system. Motivated to seek alternatives that prioritize patient well-being, he delved into exploring innovative models of care.

Dr. Bigley's philosophy revolves around truly knowing his patients on an individual level, fostering relationships built on trust and understanding. Beyond the clinic, he finds solace in the outdoors, whether it's hiking through nature's trails or nurturing his creative side in the pottery studio or garden. His dedication to holistic care and passion for personal connections define his approach to medicine.
